Introduction
We used SOPHIA to improve clinical safety at a Mental Health Trust covering 50+ sites and more than 9,000 staff. The trust faced significant challenges due to a high reliance on bank and agency staff unfamiliar with Trust policies. This led to avoidable clinical risk incidents, unnecessary costs, and time-consuming training.

How did we do it?
We digitised critical processes, creating 40 SOPs across key areas to ensure efficiency and compliance.
Challenges
The trust had a high-reliance on bank and agency staff that were not familiar with Trust policies, which caused:
Inappropriate Discharges
Avoidable clinical risk Incidents related to inappropriate discharges, patients' deterioration and falls
Avoidable costs
Unnecessary admissions, slow and paper-based processes & duplicated work
Time-consuming
Time-consuming training for new or temporary staff
Solution
The trust used SOPHIA to map out high-risk processes and digitise checklists. From the initial10 priority areas defined by the Trust, we identified 40 discrete SOPs, such as:
Pre-admission and admission pathways and digital checklists
Discharge checklists
Clinical risk assessment protocols
Record keeping SOPs
Transition pathways
SOPs for the use of medical devices
Impact
SOPHIA has improved consistency and efficiency, providing staff with access to a dynamic repository of gold-standard SOPs
Staff have access to a digital and dynamic repository of SOPs and checklists, which makes them feel more confident
Training and onboarding for new staff is safer and faster, making them more independent from day 1
SOPHIA is supporting existing QI and transformation programs, ensuring that process changes are accessible to all staff
